Clinical Effects of Tolrestat in Patients with Diabetic Peripheral Polyneuropathy

Abstract
Background:
@EN Tolrestat inhibit aldose reductase which is responsible for sorbitol pathway. The is a report of a clinical trial of tolrestat on diabetic peripheral polyneuropathy.
@ES Methods:
@EN Thirty three patients with non-insulin dependent diabetes who had sufferend from diabetic peripheral polyneuropathy for more than 3 months were eligible for this study. Subjects were administered 200§· of tolrestat once daily for 24 weeks.
Fasting
and postprandial 2-hour glucose levels, serum hemoglobin A1c, serum routine chemistry, symptoms of peripheral polyneuropathy (pain, tingling sensation, numbness, coldness, pressure symptoms, paresthesia), deep tendon reflex (biceps jerk, triceps
jerk,
knee jerk, and ankle jerk), vibration perception threshold (lateral ankle malleolus, medial ankle malleolus, first metatarsal head, and tip of great toe), and nerve conduction velocity were measured before and after tolrestat treatment.
@ES Results:
@EN The were no significant changes in fasting and postprandial 2-hour glucose levels, serum hemoglobin A1c, or results of serum routine chemistry. While the subjects sensation of cold, paresthesia, and pressure symptoms were only slightly
improved,
pain, tingling sensation, and numbness were significantly improved after tolrestat treatment. The degree of ankle jerk was significantly improved after the 24-week treatment of tolrestat. The levels of vibration preception threshold at four sites
significantly decreased after tolrestat theraphy. Motor and sensory nerve conduction velocities were not changed after tolrestat theraphy. Of 33 cases, one case experienced liver function abnormal assicated with diarrhea, and another case
developed
anorexia after 12-week treatment of tolrestat.
@ES Conclusion:
@EN Treatment with tolrestat 200§· once daily for 24 weeks significantly improved most symptoms and signs of diabetic peripheral polyneuropathy. Without severe side effects.
